Job Description
Title: Nurse Administrator – Saint Joseph Residence Reports to: Transitional Services Director
Position Description:
This full-time position is responsible for the management and supervision of program operations, staffing and supervision of all nursing and resident care activities of Saint Joseph Residence, a residential program for individuals who have been diagnosed with HIV/AIDS. The director is expected to direct and to be involved in planning, developing, implementing and coordinating all service activities, programs and special events as well as being the clinical director and supervisor of nursing, recreational and family services to the residents.
Key Tasks - Must be committed to directing and working with an integrated, intensely involved care team addressing the resident’s physical, emotional and spiritual needs; - Provide/coordinate pre-admission screening of referrals and conduct admission interviews; - Develop and implement nursing care plans based on physician’s orders and plan of care to ensure the health, comfort and safety of the patient, monitor outcomes and evaluate patient’s care - Daily documentation of the care plan activities and changes - Coordinate with various community agencies to assist residents in obtaining medications as necessary - Monitor and secure medications (especially scheduled narcotics) - Coordinate the provision of social service activities such as identification of and linkage with appropriate personal benefit programs - Coordinate resident recreational, social and leisure time activities - Administer medications per physicians’ orders - Schedule and assure appropriate staffing for 24 hours, 7 days weekly, 365 days per year; - Participate in and the development, monitoring and administrative oversight of program’s annual budget

Job Description
Title: Migration Refugee Resettlement Case Manager
Reports to: Transitional Services Director
Position Description:
The Refugee Resettlement Program provides resettlement assistance to approximately 50 refugees or asylees annually. The Case Manager is to ensure that refugees approved for admission are sponsored and offered appropriate assistance in their initial resettlement in the United States. The primary goal includes assisting refuges in achieving economic self-sufficiency in coordination with publicly-supported service providers and assistance programs. Assistance with Affidavits of Relationship are also facilitated.
Key Tasks - Ensure that case files are professionally maintained-including appropriate case notes and other updates - Assure delivery of services to refugees as per USCCB contract guidelines - Regularly monitor and review applications and AOR’s - Complete and compile service and contract reports as required - Develop and implement short and long term program goals and needs of client - Act as liaison for all refugee services with other community service providers - Communicate and coordinate assurance of incoming refugees with the USCCB - Identify housing for incoming families, coordinate delivery of furnishings, and provide orientation to housing and surrounding community - Draft assistance vouchers and monitor individual client budgets - Ensure that refugees obtain proper documentation such as social security cards and employment authorization if needed. - Ensure that refugees have received adequate cultural orientation-including basic survival skills, such as how to pay bills, manage a bank checking account, utilize community services such as the post office and library, etc. - Network with community volunteers and organizations to provide support services
Key Skills - Bachelor’s degree in Social Sciences or related field, or extensive relevant experience. - Solid planning and organizational skills - Ability to prioritize and manage multiple tasks and responsibilities and meet deadlines - Basic writing skills for general letters and meeting notes - PC literate, Word, Excel, and Outlook
